Insurance: how payment risk is shared
03 Insurance: how payment risk is shared What does “covered” actually mean for a PT visit?
Core ideas from class
- Review premium, deductible, copayment, coinsurance, out-of-pocket maximum, allowed amount and network status as separate concepts.
- Compare broad insurance designs such as HMO, PPO, POS, indemnity and high-deductible plans at a conceptual level; actual benefits vary by contract.
- Coverage, prior authorization, medical necessity and patient cost sharing are different checkpoints in a visit’s financial journey.
Think like a PT
If a patient has a $40 copay, explain why this does not necessarily tell you the payer’s allowed amount or the clinic’s net revenue.
Remember
“Accepted insurance” does not mean “no cost,” and a benefit check does not guarantee reimbursement.
Historical source note: Carrier examples and plan-specific benefit descriptions in the original lectures are historical.
