Skilled nursing: the PDPM transition
08 Skilled nursing: the PDPM transition Why does the SNF patient’s clinical profile matter beyond the number of therapy minutes?
Core ideas from class
- Understand the transition from the RUG-IV model to the Patient-Driven Payment Model (PDPM), effective October 1, 2019.
- PDPM classifies covered Part A SNF stays using separate components for PT, OT, SLP, nursing and nontherapy ancillaries, plus a non-case-mix component.
- Functional status and clinical characteristics matter. Do not equate the payment category with a prescribed intensity of skilled therapy.
Think like a PT
Two SNF residents receive the same number of PT visits but differ in complexity and function. Explain why visits alone cannot describe case-mix or medical necessity.
Remember
PDPM organizes reimbursement; it does not replace individualized clinical reasoning.
