FROM MY PT SCHOOL NOTES · MODALITIES · UNIT 4 · CHAPTER 17
How Are TENS, NMES, FES and Iontophoresis Different?
These approaches use electrical current for different purposes and should not be conflated.
At a glance
TENS
Sensory symptom modulation
NMES
Elicit muscle contraction
FES
Time activation within a task
What matters most
- TENS is commonly used for short-term symptom modulation; meaningful benefit is assessed through function and the person’s response.
- NMES aims for muscular activation, whereas FES times stimulation with a purposeful activity such as part of walking or reaching.
- Iontophoresis assists topical drug delivery and therefore adds medication and skin-specific considerations; high-voltage pulsed current is yet another waveform category.
Safety and evidence limits
Do not use a generic stimulus setting or claim implanted electronic devices are safe without device-specific review. Medication-assisted procedures require appropriate prescribing and screening.
Try a clinical decision
Scenario: A patient performs a stronger electrically assisted quad contraction but stair ability does not improve. Has functional success been demonstrated?
Reveal the reasoning
Not yet. Reassess volitional activation, exercise performance and the target task; stimulation can be an adjunct to an active plan.
