Supervision, plans of care and documentation
07 Supervision, plans of care and documentation Who can provide the service, and whose signature or oversight is required?
Core ideas from class
- Keep PT, PTA, aide, student and other personnel roles distinct. Verify federal coverage requirements and each state’s practice act separately.
- Arkansas-specific supervisory language in the historical materials does not govern every state; compare the applicable state and payer rules.
- Starting in 2025, Original Medicare allows general supervision of PTAs and OTAs by therapists in private practice for applicable services. This does not override stricter state law, other payer rules, or professional responsibilities.
Think like a PT
A private-practice PTA treats while the PT is offsite. Before calling it compliant, check the setting, current CMS rule, state supervision requirements and payer rules.
Remember
Regulatory scope, coverage rules and workplace policy are overlapping checks—not substitutes.
2026 policy reference: See the CMS Therapy Services page for current payment and documentation rules, including 2026 KX thresholds. Coverage policy is not identical across payer, setting or state.
From my class sources: 9 Supervision and policy.pdf. These are personal school notes with additional editorial context, not current legal advice, payer guidance, a coding manual, or a guaranteed reimbursement rule. Verify any current policy directly.
