FROM MY PT SCHOOL NOTES · MUSCULOSKELETAL · CHAPTER 05
Neck or Arm Symptoms: How Do You Narrow the Differential?
Arm symptoms can relate to the neck, peripheral nerves, shoulder or other systems. A meaningful examination combines what the patient reports with neurologic findings and functional tasks.
Ask what reproduces the familiar complaint
Document distribution, positional response, associated weakness/sensation change, work and sleep demands, onset and progression. Compare neck movement with shoulder/arm tasks, and target strength, reflex, sensation or neurodynamic testing to the clinical question.
Spurling-type tests are not an MRI
Your cervical lab distinguishes reproduction of familiar radiating arm symptoms from local neck discomfort with loading. A positive maneuver may support a nerve-root hypothesis, but neither a single test nor digit distribution proves a particular disc or nerve level.
Separate dizziness from cervical pain
The source collection includes dizziness and joint-position-sense materials, but neck pain alongside dizziness does not prove cervicogenic dizziness. A separate history, neurologic/vestibular differential and safety assessment are essential.
A key lecture-era limitation
The older cervical special-test lecture includes high-confidence tissue labels and positional vascular maneuvers; those are preserved as historical teaching concepts, not current diagnostic clearance tests.
External guidance and verification (separate from original notes)
Try a clinical question
Neck stiffness, hand tingling in fingers four and five, and shoulder pain: what must you not assume?
Do not assign one diagnosis from finger location alone. Consider lower cervical nerve-root, ulnar/peripheral nerve, shoulder and thoracic-outlet or medical contributions, and select tests according to objective neurologic findings and function.
